2021-2022 Registration Form

Please fill out the form below and press submit! We will send you an email back with suggested classes for your child. Please scroll down to the bottom after you have finished to pay the registration fee.  The registration fee is $30 per child ($70 max per family).  Once you respond with your choices, you will be officially registered for the 2021-2022 dance year!  If you fill out the form below, you do not need to fill out a form at the studio.  If you would like to add classes after you have registered, please email us at jdo5678@comcast.net.  We can't wait to see you in September!
