High Demand COVID Release Form

General/Covid-19 Liability Waiver

A signature is required for every participating dancer (parent/legal guardian if dancer is under the age of 18). In addition, a studio representative must sign off acknowledging that all signatures are accounts for and that all liability information has been shared with the participating dancer(s) and those accompanying them to the event. Failure to sign and submit this waiver will prohibit the dancer(s) from participating in the event. Please feel free to submit signatures in another format or use our Liability Waiver Signature Template to submit multiple pages if needed to acquire all signatures.

 

General Liability Release Statement


We will not hold High Demand Dance Competition LLC, its directors, employees or the host venue responsible for any injuries, illnesses, thefts or damages resulting from participation in this event. I hereby release High Demand Dance Competition from any and all damages my dancer(s) may sustain while participating in any activity connected with this event.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

High Demand Dance Competition has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that anyone attending our events will not become infected with COVID-19. Further, attending a High Demand event could increase you and your dancer(s) risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my dancer(s) and I may be exposed to or infected by COVID-19 by attending this event and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, HD employees and other participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my dancer(s) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my dancer(s) may experience or incur in connection with my dancer(s) attendance at an HD event.

On my behalf, and on behalf of my dancer(s), I hereby release, covenant not to sue, discharge, and hold harmless High Demand Dance Competition, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of High Demand Dance Competition, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any HD event.

By signing this waiver, I understand and agree to ensure that my dancer(s) as well as anyone accompanying them at the event will not attend if they are exempting any of the symptoms listed below or are currently quarantined at home due to possible recent exposure.

Symptoms may include, but are not limited to:

  • Fever 100.4 or higher

  • Chills

  • Cough

  • Shortness of breath or difficulty breathing

  • Fatigue

  • Muscle or body aches

  • Headache

  • New loss of taste or smell

  • Sore throat

  • Congestion or runny nose

  • Nausea or vomiting

  • Diarrhea

  • By signing below, I agree that I have read and understand all the rules, regulations and Covid-19 guidelines and procedures listed in this waiver and set forth by High Demand Dance Competition.
